Scientists are paying more attention to the connection between type 2 diabetes, metabolism, inflammation, and dementia. When blood sugar and insulin regulation are disrupted for years, the brain may also be affected. This is why researchers are now studying whether certain diabetes drugs may do more than manage glucose — they may also be linked to a lower risk of cognitive decline and dementia. The link between diabetes and dementia Type 2 diabetes does not only affect blood sugar. Over time, it can also affect blood vessels, inflammation, heart health, and the way the body uses energy. These same systems are important for brain health. Researchers have found that people with type 2 diabetes have a higher risk of developing dementia. A McGill University report notes that type 2 diabetes increases dementia risk by about 60%, which is why scientists are looking for better prevention strategies in people with diabetes. This does not mean everyone with diabetes will develop dementia. It means that long-term metabolic health matters — and improving it may also help protect the brain. Which diabetes drugs are researchers studying? Some of the most discussed medicines are called GLP-1 receptor agonists. These include drugs used for type 2 diabetes and, in some cases, weight management. They help regulate blood sugar, reduce appetite, and may influence inflammation and blood vessel health. According to the Alzheimer’s Association, large real-world healthcare datasets suggest that GLP‑1 receptor agonists may be associated with a lower dementia risk compared with some other diabetes medications, although researchers still need stronger clinical trials to confirm the effect. Another group, called DPP-4 inhibitors, is also being studied. A large McGill University study of more than 450,000 patients found that two incretin-based medication classes — GLP-1 receptor agonists and DPP-4 inhibitors — were associated with reduced dementia risk in people with type 2 diabetes. In that study, DPP-4 inhibitors were linked to a 23% lower dementia risk compared with sulfonylureas, while GLP-1 drugs showed a similar pattern but with less certainty because fewer patients were using them.
